I give The Huntington Theatre Company (HTC) permission to publish in print, electronic, social media, or video format the likeness or image of myself or my child to use/reuse, publish/re-publish, in whole or in part, individually or in conjunction with printed matter, or in composite form, and in any medium, for editorial, commercial, promotional, funding, and/or trade purposes. I release all claims against HTC with respect to copyright ownership and publication including any claim for compensation related to use of the materials.

General Waiver:
Checking this box states that you know you are participating in The Huntington Academy and that you will not sue or expect The Huntington to be legally responsible or pay for damages in the unfortunate event of personal harm or damage to personal belongings.
I grant permission to participate in The Huntington Academy and all related activities. I hereby release the staff and officers of The Huntington from any and all liability arising from my/my child’s participation. In case of accident or serious illness, I request HTC to contact my emergency contact person. If the theatre is unable to reach my emergency contact, I hereby authorize The Huntington to call the physician listed on this form above and to follow their instructions. If it is impossible to contact the physician, The Huntington may make whatever arrangement seem necessary, which we understand will ultimately be the financial responsibility of the participant.

Cancellation Policy:
This course requires a minimum of 8 students to be registered 14 days before the first session. The Huntington will provide you a full refund of your tuition if the course does not run. If you cancel with at least 21 days’ notice from the start of the course, you will receive a 75% refund. If you cancel between 14-20 days prior to the start of the course, you will receive a 50% refund. If you cancel with less than 14 days notice, you will not receive a refund.
